Your order will be placed with Ortho-Flex Saddleworks, Inc. after you have reviewed and confirmed your order. To confirm your order, please (1) complete this Sale Confirmation, (2) sign the customer satisfaction policy, and (3) complete the exemption certificate (should you qualify). (4) Then fax all three documents to 936-344-0560.

Firm Order Confirmation

Your order will be entered for available production time only after you have reviewed, signed, dated, and returned this Firm Order

Confirmation, with payment. We do not "promise" delivery dates due to the many variables associated with custom saddle making.

Each Orthoflex product goes through inspection at each major completion stage, any of which can be rejected, and could delay the

products’ completion. We will periodically give the most probable "projected" delivery date based on the order confirmation date

and/or current status of your order.

Limited One Year Warranty: Orthoflex Saddleworks Inc. (the Company) warrants its custom saddles and tack, to the original purchaser only, to be free of defects in materials and workmanship for one (1) year from the date of original (first) sale. Determination as to whether a product is defective, or is not defective, is determined solely, by the company. Freight to return a product to the company, for defective determination, is borne by the purchaser. Freight to return the product to the purchaser is borne by the company, in the continental USA only. The company will repair or replace, at it’s sole discretion, any product found to be defective by the company, for use as intended, after return to, and examination by, the company. This LIMITED ONE YEAR WARRANTY does not cover incidental damages or consequential damages caused by misuse, abuse, accident, or neglect caused by any person(s) or thing(s), purchaser’s sole remedy being repair or replacement, at the sole discretion of the company. Functional alterations of any kind, by the purchaser, dealer, or other person(s), to the product specifically void this warranty.

General Provisions and Exclusions Applicable to the Limited Warranty: Liability of Orthoflex Saddleworks Inc. shall be limited to the price of the saddle and the company shall not be responsible or liable for any incidental or consequential damages of any kind. Warranties included herein are in lieu of all other warranties, either express or implied. There are no warranties that extend beyond the face hereof. Purchaser may have additional rights by laws so made and provided. Jurisdiction with regard to any return privileges or warranty disputes shall be restricted to the laws of the State of Arkansas. This warranty shall not apply to any products that have been damaged while being used in any manner for a purpose not intended by the company.

Payment(s) Agreement(s): A minimum initial deposit of 50% of the purchase price is required with your order. Your remaining balance will be due when we notify you of the impending completion of your order. Should you fail to pay the balance due within 20 (twenty) days of notification, your initial deposit will be forfeited to the company to defray the loss incurred from the salvage sale or rework of the saddle to make it saleable to someone else. If you are paying by credit card, the following agreement applies to you: You agree to not charge back the purchase, to your credit card company, or any portion thereof, for any reason. If you violate this agreement by charging back the purchase to your credit card provider, you agree to pay the company damages of $2000, plus attorney’s fees and all costs associated with the company collection of damages, selling price, collection costs, plus the selling price and freight for the saddle.

Order Changes & Return Privileges: Once this form is returned, no changes or cancellations may be made without penalty for costs incurred by the company, if any work has commenced on the product. Since you have ordered a saddle made custom to your specifications, we do not offer satisfaction guarantees or refunds of purchase price for any reason and will not allow a return for refund. You may return for repair or replacement, as covered in our One Year Limited Warranty. We also have an excellent Customer Satisfaction Policy, which can be viewed on our web site or in the package with the product.

No warranties expressed are implied are granted in this sale by Many Blessings Farm (hereafter known as MBF) Saddles are manufactured  by Ortho-Flex Saddleworks, Inc.  and not MBF.  MBF does not repair saddles and is not in the saddle repair business. You are hereby informed that saddles are subject to heavy use and strain, and that even a carefully inspected saddle can break under that strain. You are  advised that saddle parts,  such as (but not limited to) girths, buckles, billets,  stirrups, stirrup leathers, etc. can break resulting in accident, injury, and death to the rider or observers.  You are also advised that leather parts that are cracked or worn  (such as cracked billets) should be replaced.  Saddle parts should be cleaned,  well  oiled and conditioned often, certainly after use, to prevent drying and cracking.  With your signature you  agree to accept that risk involving the purchase of Ortho-Flex saddle sold by MBF  described on this invoice and agree to  bring no claims, demands, actions or  causes of action, and/or litigation, against MBF, its owners or agents for any economic or non-economic losses for any reason including but not limited to bodily injury, death, or property damage, sustained by me or others including family members, or others to whom I may loan/sell (the saddle)  in relation to purchase of use of the saddle described on this invoice.

Shipping Damage:  If the saddle is damaged in shipping, please keep all packing materials and contact the shipper.  All shipments are insured.  It is the purchaser's responsibility to process the claim and to contact MBF regarding the damage.

(10) DAY CUSTOMER SATISFACTION POLICY

If your new Ortho-Flex custom saddle does not fit properly and is not reasonably satisfactory to you, provided you timely comply with all satisfaction policy requirements as stated below, the company will:
Produce a new replacement saddle for you, one time only, of equal or lower value, at no additional charge, except for round trip freight which is the customer’s responsibility. Changes in model, specifications, or requested upgrades for the replacement saddle, not included on you first saddle, may cause additional costs to you. After one replacement saddle is produced, this Satisfaction Policy expires and our regular One Year Limited Warranty, only, applies to your replacement saddle.
Note: This Limited Ten Day Satisfaction Policy applies only to new custom saddles manufactured by Orthoflex Saddleworks Inc. It does not apply to used saddles, "bone yard" saddles, "as is" saddles, or discounted sales sample saddles. This is not a "money back guarantee" or a "satisfaction guaranteed" policy and we do not refund your purchase price, or any portion thereof, because the new saddle was made to your personal specifications, and a readily available resale market may not exist for us to recover some of our cost.
SATISFACTION POLICY REQUIREMENTS
1. You must notify Orthoflex Saddleworks Inc., via certified mail and postmarked no later than 10 (ten) calendar days after the date of your first receipt of your saddle, of your intent to return the saddle for replacement under this "10 Day Satisfaction Policy". Messages, telephone calls, e mails, faxes, and other forms of communication, unless confirmed in writing by us, are not acceptable notice. Please ask for a Return Authorization # (RA#) which you must affix to the exterior of the saddle container. Note: Saddles received without RA#’s affixed are refused. AND
2. Within ten (10) calendar days of receiving an RA#. You must ship the saddle back to the company, at your expense and responsibility, and insured for it’s full value. We do not issue call tags for saddles being returned.  AND
3. Return the saddle in "new" condition. Should the saddle not be returned by you in "new" condition, the company reserves the right to charge you a "penalty fee" to compensate the company adequately for any resale or other losses on the returned saddle. The "as new", "penalty fee", and "reasonableness" determinations are in the sole discretion of the company. Generally speaking "new" means, but is not limited to, no stains, no discolorations, no wear spots, no scratches, no sweat marks, no horse hair, and no other unsightly foreign matter on the saddle. It is your sole responsibility to keep the saddle in "new" condition, before returning. Failure to do so, or to properly pack the saddle for shipment, will void any recourse for you, under this policy. It is suggested that you cover the saddle, including the underside, with a suitable material to prevent soiling, etc. during the first 10 days of possession.
The Customer Service Representative will inform you of the amount of freight on your new replacement saddle and answer any questions. Payment for the return freight on your new saddle must be received before your new saddle is shipped.
